Referee Documents

Our referees are integral to the success of Orlando City Youth Soccer Club.  In order to make the process as easy as possible, we have included contact information for the assignor as well as the important forms necessary for payment and documentation of games.

Payment Information

All OCYS referees must fill out and return a completed W-9 to Lsabino@orlandocityyouth.org in order to receive a payment for services.

***Any referee that has an address change is responsible for turning in a new W-9 in order for their address to be changed in system in order for the checks to be sent to the correct address.*** 

Game Reports

A FYSA game report must be completed for any OCYS GCFYSL or SSL Game. These reports are to be sent to tneidert@orlandocityyouth.org,
